Please bring the completed form with you to your scheduled appointment. 

To schedule an appointment, please contact our office during regular business hours:

Monday to Friday: 8AM – 5PM

Please bring the following to your appointment:

• Insurance Card

• Co-Pay

• Co-Insurance

• Deductible (if applicable)

• New Patient Form or arrive 20 minutes early to fill out

Appointment Cancellations: We understand that emergencies arise. Please notify us at least 24 hours in advance if you need to cancel or reschedule your appointment.

Late Cancellations: Appointments cancelled with less than 24 hours’ notice will be considered a “late cancellation” and may be subject to a $40.00 to $75.00 fee.

No-Show Policy: A “no-show” occurs when a patient fails to appear for their scheduled appointment without notifying the office. Patients who do not show up, or who arrive 15 minutes or more late, will be considered a no-show and charged a $40.00 to $75.00.
